    how to attach a document with mail form

    can any body tell me how to how to attach a document with mail form, so that on submit the form will send to my mail address as attachment,

    Read up on file uploads, and then look into using PHPMailer to handle the detail work of sending an email with a file attachment. (Or read through the mail() manual page (including users' comments) if you prefer to "roll your own.")

    i simply need an email form that have fields name, email,phone number, attachment (doc, docx and pdf files)and on submit it will send the data including attachment to my email id.
    can you please give me code for the form that i need??

    The links already posted show code examples for attachments (and the other parts you'll need to make the upload portion work as well). Perhaps you could do a couple beginner tutorials to get some of the basics and those links will make sense?

    Why don't you use some of the links in criterion's post to start with a form, show us what you have, and we can provide some help. It's not an overly difficult task, even for a beginner. You just need to get out and try a thing or two.

    i have a php career form but its not working,

    sir i have a php career form but its not working, please find it in attachment
    and please fix it so that it will send the for fields with attachment to my email address. please fix this file so it will work correctly

    well, judging by this page (http://toyotaislamabad.com/career.php) you don't have all the necessary files that whoever wrote this including (Mail.php, Mail_Mime/mime.php). That would be my first suggestion.

    that are the libraries include in it.
    can you please find any other code that will send the fields as well as the attachment, i am a new so i am unable to find that why i ca here for professionals to help me

    If you don't want to make it yourself, you can use php form creator
