Need help!! Please!!

I hope I am in the right place. I am very new at this scripting stuff, so I am hoping someone can help me out. Here is what I am looking to do, I need to be able to direct a visitor to a specific page on a site depending on what is entered in a box. For example:

If a visitor enters in a box that they want to see a certain claim, they enter the claim number hit submit and it takes them to www.examplesite.com/claimnumber

Does this make sense?
