PHP form that sends files to my server, and the info to my email

So basically how can i have a form that sends files to my server, and the other info to my email after they hit the submit button.

I am looking to have the form similar to this with the file upload built into the form- http://www.chiefsigns.com/resources/

This has been driving my crazy! any help would be greatly appreciated, or even links to tutorials on this.
