Web form data empty or null

im having problem with the data sent to email as null or empty, anyone know why this is? attached are the files im working on, hopefully some experts can give me some advices on this
3 files
one html
one js (form calculation)
one php (send results)

in cal.html fix this:
action="send.php" metdod="post"
should be

action="send.php" method="post"

I did not check if the rest of the fields is not messed up.
Fix this and see what you get.
