Hello Guys,

I'm not sure if this is the right section. But i'm having difficulties in creating an Employee Attrition Form using Sharepoint 2003. This what we have in our company and I have to deal with it.

What I want to have is a conditional menu like below sample:

Personal
going back to school
pursue different career
better opportunity


Medical
Health related
operation
maternity

Others
company related
others

What I want to do is when a supervisor files a resignation for his subordinate, the first level of menu would be Personal, Medical, Others.
then after choosing one, a second level menu will show based on the first choice.

What we have now is 2 menus without restrictions, if someone chose Medical, the selections from Personal and Others will still be there. It makes my analyst do manual checking to everything filed if they are filed correctly or not.

Is this even possible? thanks!
