Strange problem with dynamic checkbox buttons?

Alright so nobody we know can figure this out. On our page we have different checkbox options that make different content appear. What's bizarre is that only one of the sections works properly and all the others do not. What's even more strange is that the section is not at the beginning of the page like I would expect it to be.

Here is the one that works:
    http://pastebin.com/c5J6ycTJ

Here is a section that does not work:

If anyone has the slightest idea of why this is not functioning I would be absurdly thankful. If any more code is required to see I can do that too. Another "problem" I suppose would be that the submit buttons are not actually needed nor used but that is something I care far less about. If someone wanted to help me with that too though, I would be super stoked!

I pasted your code in js fidddle. To show/hide the sections you mention you should use javascript or some framework like jquery. Where is that code?

Honestly I'm not sure where the js is located on that page in particular but its there somewhere. I'm still kind of new to web design so I'm learning a lot as I go. Maybe the full page source code will help out:

Please, post your code in www.jsfiddle.net, run it, save it and paste here the url in jsfiddle after saving it. Say exactly what doesn't work or what you expected to happen.
