Help on the best way to make a quote form

Hi i own a fencing company in surrey and im looking to make a form that would let my online customers pick what type and size of fencing they would like then they enter the length they want and the form calculates how much it will cost not sure which way would be the best ive searched for templates with no luck any help would be great! below is how i want to lay it out....

Post and gravel board - select ever .......
    Timber
    Concrete

Type of fencing – timber - if timber these are the prices for height of fence at 6ft long
    Lap fence panel – 3ft high – 117.50
    Lap fence panel – 4ft high – 114.50
    Lap fence panel – 5ft high – 112.50
    Lap fence panel – 6ft high – 109.50

+ end post – 66.50 - the prices above only have been priced for 1 post so need to add the 66.50 to the final price

Type of fencing – concrete - same as above but concrete
    Lap fence panel – 3ft high – 119.50
    Lap fence panel – 4ft high – 120.50
    Lap fence panel – 5ft high – 121.50
    Lap fence panel – 6ft high – 122.50
    + end post – 76.50

    Thanks again!
