    you can try with this code

    <html>
    <head>
    <title>Process the HTML form data with the POST method</title>
    </head>
    <body>
    <form method="POST" action="process.asp" name="form1">
    <table width="70%" border="0" cellspacing="0" cellpadding="0">
    <tr>
    <td>name:</td>
    <td colspan="2"><input type="text" name="name"></td>
    </tr>
    <tr>
    <td>email:</td>
    <td colspan="2"><input type="text" name="email"></td>
    </tr>
    <tr>
    <td>comments:</td>
    <td colspan="2"><textarea name="comment" cols="40" rows="5"></textarea></td>
    </tr>
    <tr>
    <td>&nbsp;</td>
    <td colspan="2"><input type="submit" name="Submit" value="Submit"></td>
    </tr>
    </table>
    </form>
    </body>
    </html>

    I'm thinking you could simply execute that plugin whenever your button is clicked.

    Thanks for the code so incase i need to send the filled information to an email. Where do I place the page link and my email so that when I click submit all the dat filled goes to my email?

    Also is it possible to save the data filled in an HTML form to a local machine just as you can save word document?

    Thank you once again.

    In the action field of the form, set the action as “mailto:youremailaddress” for example:

    <form action="mailto:myforms@mydomain.com">

    this can act different in different browsers though. the best way to set this up is with php. in my opinion
