    <title>Form</title>
    </head>

    <body>
    <form method="post" action="getform.php">
      <p>
        <select name="type" size="1" id="select">
          <option value="site" selected="selected">Dive Site</option>
          <option value="boat">Charterable Boat</option>
          <option value="slip">Slip</option>
        </select>
      <p>Name of Site:
        <input name="dname" type="text"><br>
      <p>Latudude:
      <input name="lat" type="text">
      <br>
      <p>Longditude:
      <input name="lng" type="text">
      (If you don't know leave blank and put a detailed explination of the location in the discription)<br>
      <p>Discription:<br>
        <textarea name="desc" rows="15" cols="41"></textarea>
        <br>
      </p>
      <p>Your name:
      <input name="yname" type="text">
      <p>
        <input type="submit" />
        <input type="reset" name="Reset" id="button" value="Reset" />
      <br>
     </form>
    </body>
    </html>
    and submit it via email using php. I want the from line to simply be form and the subject to be the "dname" value The rest of the field I want to be included in the message body.

    Not sure if this is what you want but... you must think about injections as well research code that will help prevent them... Its a start

    PHP Code:
    $email $_POST["email"];
    $message $_POST ["message"];
    $meemail ="your email";

    $email_from $meemail;//<== update the email address
    $email_subject "Message from $dname";
    $email_body "your messgage".

    $to "email address";//<== update the email address
    $headers "From: $meemail \r\n";
    //Send the email!
    mail($to,$email_subject,$email_body,$headers);
    //done. redirect to thank-you page.

    ?>

    I can get that much so far but I either get errors or only the first item gets submitted how do I get the "$email_body" to include multiple form fields?

    PHP Code:

    $email_body
    "We have received a new message  \n".

    "Here are the details:\n
    Name:
    $firstname $surname \n
    Email:
    $email \n
    Message:
    $message \n
    Contact information:
    $contactinfo \n"
    Thats how to include multiple form fields

    If the example doesn't work you need to post the email code as well so we can have a look at it

    I keep receiving a syntax error on line 9. But the $email_body part is error free now.
    PHP Code:
    <?php
     $to
    "diver@nicholasmanuel.net"

    $subject $_REQUEST['dname'];

    $email_body "We have recieved a new site submission \n

    $type \n

    $lat$lng \n

    $desc \n

    $yname"

    $headers "From: divesites@nicholasmanuel.net" ;

    $sent mail($to$subject$email_body$headers) ;
     if(
    $sent
     {print
    "Your dive site was successfully submitted"; }
     else
     {print
    "We encountered an error submitting your dive site"; }

    ?>

    Change this:

    PHP Code:

    $yname

    To This:

    PHP Code:

    $yname
    \n".
