    Missing Contact Form data

    Hi.

    I'm building a website using Dreamweaver CS6. I added a form on the Contact page and got the PHP script ready to send the data entered to an email.

    When I test it out, the email arrives with the subject line filled out and the identifying text (Name: , Email: Subject:, Message: ), but there is:
    1) no email in the From field, and
    2) no Name, Email, Subject, or Message data that was entered.

    Where's the best place to start looking for the problem: Dreamweaver or the PHP script? And what's the best way to do that?

    Thanks in advance.

    Here's the code.

    HTML Code:
       <form action="ndpcontactformprocessor.php" method="post" enctype="text/plain" name="contact" target="_top" id="contact">
          <table width="75%" border="0" align="center" cellpadding="2" cellspacing="1" class="Detail" summary="Table used for form layout">
            <tr>
              <th colspan="2" class="Detail" scope="col">To contact us, please complete this form.</th>
            </tr>
            <tr>
              <td width="23%" class="Detail"><input name="required" type="hidden" id="required" value="first_name, last_name, email" />
              Name *</td>
              <td width="77%"><input name="name" type="text" id="name" size="40" maxlength="100" /></td>
            </tr>
            <tr>
              <td class="Detail">Email *</td>
              <td><input name="email" type="text" id="email" size="40" maxlength="100" /></td>
            </tr>
            <tr>
              <td>&nbsp;</td>
              <td>&nbsp;</td>

    It is never a good idea to use HTML name tags that use HTML identifiers, eg name="name", try using name="username" or name="person"

    I changed the form type to enctype="application/x-www-form-urlencoded" and it worked fine.
