    Showing form and generating text

    I have a form I am working on. I want to be able to do a couple things with it and I'm having trouble finding information. The form is below. What I would like to do are a few things.

    Number one, I'd like to have the option to show/hide additional instances of the "Remote Maintenance" div below the current one. The first one should always be visible, but I'd like the option of having a duplicate form, from "Server Name" to "Updates". This is in case sites I'm working on have more than one server.

    Also, I'd like the option to show/hide the OM div, however it does not need to be hidden by default.

    I'd like to have a reset button under each div. That would be below "Servers Being Rebooted", "Updates" for Server and OM.

    Finally, I'd like to have a blank, uneditable text box below the forms with a button that would take all the form data and display it as text. I would like the text in the box to be selectable, so the text can be copied/pasted. And maybe a button that would automatically copy the text in the box to clipboard.

    I haven't really found anything helpful. This is the first time I'll be doing anything with java.

    Code:
    <!DOCTYPE html>
    <html>
    <body>
    <form action="" form id=”RemoteMaintenanceForm”>
    <div id="Servers">
    Server Maintenance Notes: <input type=”text” name=”ServerMaintenanceNotes”><br>
    <p>
    Server(s) Being Rebooted: <input type=”text” name=”Server(s)BeingRebooted”><br>
    <p>
    </div>
    --------------------------------------------------
    <div id=RemoteMaintenance>

    I would also like to be able to add or subtract the copies of the forms indefinitely.

    I found some code that does what I want it to do, as far as replicating the form goes. The only problem is, I don't know how to get it to work with my form. Here's the code:

    Code:
    <html>
        <head>
    <script src="http://ajax.googleapis.com/ajax/libs/jquery/1.3/jquery.min.js"></script>
    <script type="text/javascript">
            $(document).ready(function() {
                $('#btnAdd').click(function() {
                    var num     = $('.clonedInput').length;
                    var newNum  = new Number(num + 1);

                    var newElem = $('#input' + num).clone().attr('id', 'input' + newNum);

                    newElem.children(':first').attr('id', 'name' + newNum).attr('name', 'name' + newNum);
                    $('#input' + num).after(newElem);
                    $('#btnDel').attr('disabled','');

                    if (newNum == 5)
                        $('#btnAdd').attr('disabled','disabled');
                });

                $('#btnDel').click(function() {
                    var num = $('.clonedInput').length;

                    $('#input' + num).remove();
                    $('#btnAdd').attr('disabled','');

                    if (num-1 == 1)
                        $('#btnDel').attr('disabled','disabled');
                });

                $('#btnDel').attr('disabled','disabled');
            });
        </script>
    </head>

    <body>

    <form id="myForm">
        <div id="input1" style="margin-bottom:4px;" class="clonedInput">
            Name: <input type="text" name="name1" id="name1" />
        </div>

        <div>
            <input type="button" id="btnAdd" value="add another name" />
            <input type="button" id="btnDel" value="remove name" />
        </div>
    </form>

    </body>
    </html>
    I just can't seem to get it to work. What do I need to change?
