    HTML Form to SQL Database

    I have a Form:

    <form action="reminder.php" method="post">
    		Name: <input type ="text" name="name" value="" /> <br />
    		Name: <input type="text" name="customer" /><br />
    		Email: <input type="text" name="email" /> <br />
    		Phone Number: <input type="text" name="phone" /> <br />
    		Vehicle Make/model: <input type="text" name="car" /> <br />
    		Registration: <input type="text" name="registration" /> <br />
    		Date <input type="date" name="date" /> <br />
    		<br />
    		<input type="submit" name="submit" value="Submit" />
    I have set the action to reminder.php which has all commands and relevant "stuff". There is also another file named "databases" which has the database name, username, password, etc...

    How can I link the HTML form so the contents publish to the database?

    The code inside that reminder.php file will need to perform the action of getting the data from the form and storing it in the database. What does the code look like in that file?
