Question PHP displaying items multiple times?

I have attached an image which help displays the problem I am having. The website is part of WordPress and all of the pages are designed in .PHP. In short our logo and login are repeating after recent updates. I can't locate anything wrong with the code and was wondering if anyone else is having this problem or knows how to fix the issues.

Your help is appreciated. Thank you.

    A few q's-
Are the doubled items the result of a printed MySQL query?

I guess my question is why do think it's PHP that's causing it?

What theme are you using, and is it custom?

The logo could be as simple as background: no-repeat; in the css.

I've already checked the no-repeat; and there doesn't seem to be an issue there; so, I think it is .php. It is a custom theme.

Well, it's going to be pretty difficult for anyone here to debug your PHP code when we can only see a screenshot of the result.

I'll take my best psychic stab at it, and guess that there are two places in your code that are include()-ing/require()-ing a file that displays the page header.
