    Form Help

I wrote a simple html form that creates cases in Salesforce. I need a javascript statement in the form to send the user to a different thank you page once submitted.

Example: For type I need the user to select between types

    <label for="type"><strong>Type</strong></label>
    <select id="type" name="type"><option value="">--None--</option>
    <option value="Website Issue">Website Issue</option>
    <option value="Magazine Issue">Magazine Issue</option>

If the user selects Website issue and submits the form they get sent thanks1.html
If the user selects Magazine issue and submits the form they get sent to thanks2.html

I have been working on this for a week with little success and was hoping I could get nudged in the right direction here.

Thanks in advance for any help
