    Did you look into creating your complete form at runtime with the click of a button by using DOM.
    That way you do not need to hard code form in HTML.

    ooh ooh ooh BRAIN ATTACK! Why don't I just save the javascript as a .js file and then link to it inside the submit() function?! Wooohoooo!!!!!!
