    Creating dynamic intake form

    I'm not sure where the right place for this question is, but I'll start here. I want to create a web intake form, similar to a wizard, which will dynamically adapt intake questions based on past responses. The end result should be that the data gets deposited in a database at some point. One example I really like is Postmates Fleet web intake form for new Fleet members (fleet.postmates.com for reference). I used SiteStacks to disambiguate the tech stack, and it appears it's mostly written out of HTML5. I'd like to understand the best way to re-engineer a similar form for my business. Thanks, in advance, for the advice.

    I thought that was an interesting use of the object and forEach() function to display.
    Neat!

    What bothered me was the way it abruptly stopped with a qa[] undefined error.
    I modified you program slightly to avoid this.
    I also added a 3rd initial question to test my modifications and understanding of your logic.

    Don't know how I would personally use it, but at least I think I better understand your post!
