combo/list box questions

Never though i would have so much touble figuring this out but I cannot seem to find what I am looking for.

I need to create a list box type input that allows the user to select more 1 or more items at the same time. The amount of information that will be eneted into each box will change depending on what the database loads. Does anybody have a good howto on how to accomplish this and how to handle submission.

so the page would look something like this

field1      choice1
            choice2
            choice3
            etc

the idea is that the choices will be linked to field 1 and the user my choose a single value or all of them or a few of them.

Can this be done and does anybody understand what I am trying to do.

Press 'Shift' or 'Ctrl' for multiple selection

ok that works fine but how do i handle server side? If the user chooses 2 values and they both have the same name, how do i access each piece of information?
