    Form Problems

    Greetings Everyone.

    I'm new to the boards and in need of a little guidance. I'm having trouble with a form. ( http://www.namastesalonandspa.com/survey.html ). The trouble starts in the last section of the form, in the rows of radio buttons.
    The first section ( your appointment ) works fine. The next area, (your arrival ) does not. If you make a selection in one area then move to the next row, the previous choice dissappears. If someone could take a look and what I did wrong, I'd be much obliged!

    hi, the name of the options have to be the same if they are options to a questions, but in ur case, all the options are of the same name. Hope it helps.

    Also remove spaces from names: http://www.w3.org/TR/html4/types.html#h-6.2

    thanks for responding!

    Im still not following on how this would affect the problem i'm having. I realize the "name" is the same in all of the input tags, I wanted it that way because the radio buttons provide five different responses to the same question. Ill try removing the spaces in the "name" . Thank for your time!

    Each group of radio buttoms needs to have it's own name
    Finishing Touch group has it's own name, Review of Experience group has it's own name, etc.
