I am very very very very new to this MySQL "thing" so please answer this as simple as posible. haha. I know what you might say..."before you do anything read a book about mysql" but all i want to do is to set it up.

I have a cpanel website and i want to set up my own guestbook. I have all the files for the guestbook uploaded in to my directory and it has a installation page....the only thing is i do not know what to put in the form box's.
