Favicon displays on some pages, not others

On my website, favicon.ico displays on all pages using FF browser but if I use Opera or Chromium it only displays on a few pages. Clearing the cache doesn’t help. All of my pages have the same code in the header:

[code] <link rel=”icon” href=”favicon.ico” />[/code]

The favicon is in the root folder of the website along with the webpages. I’ve tried using several variations of the favicon code such as:

[code]<link rel=”icon” href=”favicon.ico” type=”image/icon”/>
and
<link rel=”shortcut icon” href=”favicon.ico” type=”image/icon”/>[/code]

as well as changing the type in both cases to type=”image/x-icon”
Can anyone suggest a solution?
