Hi all,
I want to create a form on a website that will have a series of questions. Ideally I’d like to have the questions over multiple “pages” eg
Page 1 = name and contact details
page 2 = previous medical conditions
page 3 = current medical conditions
Page 4 = current lifestyle
Page 5 = treatment questions
Once a user clicks submit, I’d like to compile all the answers in a word document, then have that word document emailed to an address.
ideally naming the word document “<<First Name>> <<last name>> pre appointment questionnaire”
im completely new to all of this so where would be the best place for me to start understanding how I do this.
I appreciate I’m well out of my depth here but I like to learn and I really want to try to make this work before I give in and find someone to do it for me.
